Introduction

Welcome to Plexus Corp Pty Ltd (ABN 18 644 909 740) trading as Plexus Personal Training. Before you begin we would like to give you some basic information about our services and inform you about the risks, fees and how we will manage your personal information. This will help you understand more about the services we provide including your rights and responsibilities as a client.

Purpose

Plexus Personal Training provides personal training and other services (e.g. consultations with Accredited Practising Dietitians) intended to improve your physical fitness, health and wellbeing.

The Basics

If you participate in our physical training services, our qualified personal trainers will seek to provide you with exercise programs and instructions that are suited for your goals, preferences, and physical capabilities. You should know that:

  1. Physical training sessions are usually 45 minutes duration, commencing promptly at your scheduled start time.
  2. All sessions are one-on-one unless otherwise advised or agreed.
  3. Physical touching may be required in order for you to have proper positioning and technique for your body and/or to assess your body’s reactions to certain activities.
  4. You must inform your physical trainer of any injuries, diseases, conditions, medications or other factors which may affect your ability to perform, or safety whilst performing, physical training.

If you participate in our diet and nutrition services, our Accredited Practising Dietitians will seek to provide you with feedback and advice in relation to your diet and nutrition that aligns with your goals and preferences, as well as any dietary restrictions you have. You should know that:

  1. “Initial” dietetic consultation sessions are typically 60 minutes. “Follow up” dietetic consultation sessions are 30 minutes. All sessions commence promptly at your scheduled start time.
  2. All sessions are one-on-one unless otherwise advised or agreed.
  3. These services are usually conducted remotely via teleconferencing / video-link (e.g. Halaxy Telehealth)
  4. You must inform your dietitian of any allergies, diseases, conditions, medications or other factors which may affect your diet or nutrition / nutritional needs.

If you wish to participate in any of our other services, you can request specific information about what to expect from us at any time.

Risks

Physical Training Sessions

Physical training carries some inherent risks. For example:

  1. There are chances that during the period of training session, you may experience adverse effects such as dizziness or fainting. In most cases, this is due to fluctuations in blood pressure. In rare cases, it may lead to a heart attack or stroke which may be a cause of death.
  2. Physical training, or specific exercises, may directly or indirectly cause or contribute to injuries that may involve the joints, ligaments, muscles, and tendons.

Plexus Personal Training will take all responsible precautions to reduce risk however, it is your responsibility to ensure you are and continue to be aware of your physical abilities and limitations whilst participating in physical training. You should provide feedback to your personal trainer on your physical condition and experience regularly before, throughout and after every session.

It is your responsibility to immediately stop any exercise or activity which makes you feel pain or discomfort beyond what is normal for you.

You should always consult your medical practitioner if you have questions or concerns about any pain, discomfort, injury, condition or other factor which is affecting or may affect your ability to participate in physical training.

Dietetic Consultation Sessions

Dietetic consultation sessions carry some inherent risks. For example:

  1. Your dietitian will be unable to make recommendations in accordance with any allergies, diseases, conditions, medications or other relevant factors if you do not notify them of these.
  2. It is important that you notify your dietitian of your current medication because some foods or food groups can interfere with the safety or efficacy of some medications.
  3. Changes in your diet and nutrition as well as your overall health may change your dietetic needs and/or preferences from time to time. You need to keep your dietitian informed in order for these needs and or preferences to be met.

Plexus Personal Training will take all responsible precautions to reduce risk however, your participation is voluntary and you have the right to withdraw your consent or cease sessions. It is your responsibility to ensure you continue to monitor your response to changes in your diet and nutrition, as well as your overall health and keep your dietitian informed accordingly. You should provide feedback to your dietitian regularly during every dietetic consultation session.

Dietetics advice is not a substitute for the diagnosis, treatment, or care of disease by a medical practitioner.

Telehealth

Dietetic consultation sessions will be ordinarily conducted remotely via teleconferencing / video-link (e.g. Halaxy Telehealth). You will receive an invitation to join your telehealth session via email to the email address provided on initial registration (or as updated by you at any time). Plexus Personal Training may choose to send a text message to remind you of your telehealth session.

For all sessions conducted via telehealth, it is your responsibility to;

  1. Join your dietetic consultation session at the scheduled start time,
  2. Ensure you are in a private location where either there are no other people present or where other people cannot overhear your session (without your consent),
  3. You have adequate videoconferencing equipment and suitable internet connection to participate in the entire session without disruption including that you are clearly visible and audible at all times. Sessions which are interrupted or terminated due to technical disruptions will not be refunded or rescheduled.
  4. Ensure that your email address nominated with Plexus Personal Training is a secure and safe address that only you have access to.

Fees

Physical Training Sessions: Fees are payable through an up-front package via invoice, using a nominated Debit or Credit Card, or a direct bank deposit, or alternatively, through a weekly direct debit. New invoices will be sent when there are 2 sessions remaining in credit. For clients on a direct debit arrangement, funds will be deducted from your Debit or Credit Card in advance for the upcoming week every Monday.

Dietetic Consultation Sessions: Fees are payable on a per-session basis through the online booking platform. Payment information may be stored in accordance with the terms and conditions of the booking platform.

  • All fees paid are non-refundable.
  • In the case where fees are unpaid, training will not continue until fees are paid.
  • You may cancel the service at any time.
  • Plexus Personal Training reserves the right to cancel or reschedule training sessions as necessary or required. Training sessions cancelled by Plexus Personal Training will be credited to you.

Cancellations

  • Plexus Personal Training requires 24 hours’ notice for cancelling or rescheduling scheduled training sessions or dietetic consultation sessions. Sessions cancelled or rescheduled with less than 24 hours’ notice will be forfeited unless we deem the reason to be under extenuating circumstances.
  • In the case of 2-1 training sessions, the Client acknowledges the higher pricing, reflecting the need for additional studio space and privacy. It is understood that, irrespective of the attendance of one participant, the session will be charged according to the agreed-upon 2-1 pricing structure.
